By booking or participating in Botox & Restore Day at New You Medical, you agree to the following terms and conditions.
1. Event Eligibility and Booking
- Botox & Restore Day is a limited, appointment-only event.
- Appointments must be scheduled in advance. Walk-ins are not guaranteed.
- Appointment availability is limited and offered on a first-come, first-served basis.
- A valid form of payment must be on file to reserve an appointment.
2. Required Intake Forms
- All patients must complete required intake forms and consent documents prior to their scheduled appointment.
- Failure to complete intake forms in advance may result in appointment delay, rescheduling, or cancellation.
3. Medical Eligibility
- All treatments are subject to medical evaluation and provider approval.
- New You Medical reserves the right to decline or modify treatment if medically contraindicated.
- Results may vary based on individual anatomy, medical history, and response to treatment.
4. Treatment Details
- Injectable treatments are administered by qualified medical professionals.
- The specific botulinum toxin products used during Botox & Restore Day include Botox and Xeomin.
- Dosage, injection sites, and treatment plans are determined by the provider at the time of service.
- Complimentary services, including Vitamin B12 injections, are provided only if medically appropriate and approved by the provider.
5. Pricing, Discounts, and Promotions
- Event-only pricing and promotions are valid only on the Botox & Restore Day event date.
- Tiered discounts, free units, rewards, and promotional details may vary and can be reviewed with clinic staff on the day of the event.
- Promotions, discounts, free units, and rewards cannot be combined with other offers unless explicitly stated.
- All promotions are subject to availability and may be modified or discontinued at New You Medical’s discretion.
6. Referral Bonus Policy
- Referral bonuses apply only when the referred patient receives more than 30 units.
- For treatments under 30 units, a $10 discount will be applied instead.
- Referral discounts have no cash value and are non-transferable.
7. Rewards, Free Units, and Giveaways
- Free units, reward points, raffles, and giveaways are limited in quantity and availability.
- Rewards and free units have no cash value and cannot be redeemed for cash.
- Eligibility details and tier qualifications may be confirmed with clinic staff at the time of service.
8. Cancellations and No-Show Policy
- Cancellations must be made at least 24 hours prior to the scheduled appointment.
- No-shows or same-day cancellations will result in a $50 no-show fee, charged to the payment method on file.
- Failure to attend may result in forfeiture of promotional pricing or event eligibility.
9. Results and Expectations
- Cosmetic outcomes are not guaranteed.
- Full results may take several days to two weeks to become visible.
- Touch-ups, additional units, or follow-up treatments are not included unless explicitly stated.
10. Photography and Marketing
- Before-and-after photos may be taken for medical documentation.
- Marketing use of photos will occur only with written patient consent.
- Declining photography does not affect eligibility for treatment.
11. Limitation of Liability
- Patients acknowledge the inherent risks associated with injectable aesthetic treatments.
- New You Medical is not responsible for adverse outcomes related to undisclosed medical conditions or failure to follow post-treatment instructions.
12. Event Changes and Modifications
- New You Medical reserves the right to modify, reschedule, or cancel the event if necessary.
- Patients will be notified of any changes and offered appropriate alternatives.
13. Acceptance of Terms
- Booking and participation in Botox & Restore Day constitutes acceptance of these Terms and Conditions.
- Terms are subject to change at the discretion of New You Medical.
